A smooth 10DLC approval process starts with one thing: a correctly completed registration form. Carriers review every detail you submit, and even small mistakes can lead to delays, rejections, or limited message delivery.

Below is a simple guide explaining exactly what your form should include so your campaign gets approved quickly and without issues.

1. Use Accurate Business Information

Your business name, address, website, and EIN must match official records.
Carriers compare your form to public data, so everything should be:

  • Consistent
  • Exact
  • Up-to-date

If something doesn’t match, they flag it — which slows down approval.

2. Clearly Explain Your Use Case

Carriers want to know what type of messages you send.

Examples of clear use cases:

  • Appointment reminders
  • Customer support updates
  • Order and delivery notifications
  • Promotions or marketing messages
  • OTP / verification codes

Avoid vague phrases like “general messaging.” The more specific, the better.

3. Provide Realistic Message Examples

This is one of the biggest reasons forms get rejected.

You must provide 2–3 actual examples of messages you will send.
They should look real — not placeholders.

Example:

“Hi John, your appointment with CleanCare Dental is confirmed for Tuesday at 4 PM. Reply YES to confirm or NO to reschedule.”

Fake example that will get rejected:

“Test message.”

4. Describe Your Opt-In Method

Carriers require proof that customers agree to receive your texts.

Your form should explain:

  • Where customers opt in (website, checkout form, verbal agreement, keyword, etc.)
  • How consent is captured
  • What kind of messages they expect

If carriers don’t see a valid opt-in method, the application will be denied.

5. Ensure You Support STOP & HELP

All SMS campaigns must support:

  • STOP → unsubscribes immediately
  • HELP → gives customer support info

Your form should confirm you support standard compliance keywords.

6. Have a Live Privacy Policy & Terms Page

Your website must include:

  • A visible Privacy Policy
  • Terms & Conditions
  • SMS disclosure if collecting phone numbers

Carriers verify these pages manually.

7. Check for Consistency Across Everything

Your:

  • brand name
  • domain
  • message examples
  • use case
should all match and make sense together.

If something looks off, carriers assume risk and slow down approval.
